Medical forms and fitness assessments for recreational or private activities

We are often asked to complete forms or provide medical assessments for activities such as: skydiving, scuba diving, marathons, or seatbelt exemption requests.

These requests fall outside the scope of NHS care. As a GP practice, we are not indemnified or resourced to assess patients’ fitness for private, recreational, or non-essential activities, and we are therefore unable to complete forms or provide medical opinions for these purposes.

If you need to share information about your medical history with a third party:

  • Patients with online access to their records can access and share their medical summary or problem list directly.
  • You may request a free printout of a brief summary of your medical records by contacting the admin team via our website.
  • If you require a letter stating factual information about a medical condition (or lack of), we may be able to provide this for a fee. Please note this will include facts only and will not include an opinion on your fitness to participate in any activity.

If you require a formal assessment of your fitness to take part in an activity, you are free to seek the opinion of a relevant private specialist.
