We have recently reviewed our proxy access procedures to ensure they continue to protect your privacy and meet the latest guidelines. As a result, we need to make some changes to how proxy access is managed.

What Changes Are Being Made?

    We will be reviewing all current proxy access arrangements to ensure they are up-to-date and appropriate.
  2. Updated Consent Forms:
    We will require updated consent forms for anyone who wishes to have proxy access. This ensures that we have your explicit permission for someone to access your records.
  3. Enhanced Security Measures:
    We are implementing additional security measures to protect your information and ensure that only authorised individuals have access.

What Will Happen Next?

  1. We will be contacting patients that currently have proxy access on their records and request a new application form is completed. It is important that these forms are completed and returned to us. Failure to complete this could result in your proxy representative being unable to complete tasks such as ordering your repeat medications.
  2. Once we have updated all of our current proxy access accounts, we will publish the application forms on our website for anyone wishing to apply for a new proxy access account.
