The Practice runs a long term condition monitoring programme for the following diseases:
- Diabetes
- Asthma
- COPD (chronic lung disease)
- Heart Disease
- Kidney Disease
- Rheumatoid Arthritis
- Demention
- Hypertension
Patients are invited by SMS, email or letter from the practice to come to an appointment with one of our practice nursing team. We send out the invitation in the month of your birthday. So if you were born in May and have a long term condition, an invitation will be sent to you in May each year asking you to attend the practice.
You should then ring patient services on 01869 338611 to book an appointment with the practice nursing team for your long term condition review.
The invite will also tell you if you need to have a blood test in advance of coming for your appointment, and whether you need to bring a urine sample with you.
